On the 'Lessons' tab in your course select the 'Add Content' link

Choose 'Quiz'

Give your quiz a name

Access Settings

When selecting "Scramble the order of choices on questions," please be careful if adding questions with choices such as "all of the above, none of the above, or A and C," etc.
Use the "advanced settings" in the question editor for these question types, then select the "never scramble" option. If this is not done, the response choices will not be in the correct order and your students will become confused.
